You must complete/print clearly the below:

• Full name of the applicant
• Full home address
• Phone number
• Email address
• Date of birth
• Sex
• Reason for the request

Certified copy of identification (RCMP requirement):

Application Service Fees

• You will need to present two original Government-issued photo identification to BLS International for verification
• They will photocopy, sign and stamp that they are true copies of your identification (ONLY International locations)

Cover Letter

• Your cover letter must be typed, not handwritten.

You must complete/type clearly the below application information:

• Home address in full
• Email address
• Date of Birth (YYYY/MM/DD)
